New Patients

Those who are currently residing or planning to relocate within our practice vicinity can apply to be a patient of the health centre.

You can visit the clinic to complete the registration process and fill out the new patient questionnaire.  Additionally, you may book a short medical assessment with our Healthcare Assistant.

When registering, you must provide photographic evidence of your identity and residency (such as a utility bill or bank statement). 

Please be aware that we require the registration forms to be fully completed prior to us registering you as a patient.
